We are accepting dental fee schedules from all 50 states, including Puerto Rico and Guam.  Upon completion of the fee survey, you will receive a promotional discount code for a $50 discount on any NDAS book or software.  The online survey form will prompt you for your standard/list price per dental code.  Do not provide contracted fees or medicaid.  Survey participants may remain anonymous but a zip code is required.
